ARM: prepare for removal of a bunch of <mach/memory.h> files
When the CONFIG_NO_MACH_MEMORY_H symbol is selected by a particular
machine class, the machine specific memory.h include file is no longer
used and can be removed. In that case the equivalent information can
be obtained dynamically at runtime by enabling CONFIG_ARM_PATCH_PHYS_VIRT
or by specifying the physical memory address at kernel configuration time.
If/when all instances of mach/memory.h are removed then this symbol could
be removed.

This achieves two goals:
1) Get rid of omap_uart_v2p() and omap_uart_p2v() which were the last users
of PLAT_PHYS_OFFSET.
2) Remove the probing of the M bit in the CP15 control reg and make
the access to the .data variables completely position independent.
There is a catch though: the busyuart macro needs to know where the LSR
register is which might be at a different offset depending on the hardware.
Given that this macro is given only two registers and that one of them
must be preserved, the trick is to always pass the LSR register address
around, and deduce the base address for the THR register by masking out
the LSR offset in senduart instead.

ARM: add an extra temp register to the low level debugging addruart macro
Some platforms (like OMAP not to name it) are doing rather complicated
hacks just to determine the base UART address to use. Let's give their
addruart macro some slack by providing an extra work register which will
allow for much needed cleanups.
This is basically a no-op as this commit is only adding the extra argument
to the macro but no one is using it yet.

xen: PV on HVM: support PV spinlocks and IPIs
This change replaced the SMP operations with event based handlers without
taking into account that this only works when the hypervisor supports
callback vectors. This causes unexplainable hangs early on boot for

ARM: mxc: Remove setting of consistent dma size
All in-tree MX boards using video use memblock_* functions to get their
coherent dma space for the camera. So there is no need to increase
CONSISTENT_DMA_SIZE beyond the default 2MB and we can simply remove the
defines which do this.

DMA region above it's default value of 2MB. It must be called before the memory

ARM: introduce atag_offset to replace boot_params
The boot_params member of the mdesc structure is used to provide a
default physical address for the ATAG list. Since this value is fixed
at compile time and sometimes based on constants such as ARCH_PHYS_OFFSET,
it gets in the way of runtime PHYS_OFFSET and CONFIG_ARM_PATCH_PHYS_VIRT
usage.
Let's introduce atag_offset which should contains only the relative
offset from PHYS_OFFSET instead of an absolute value, in preparation
to move all instance of boot_params over to it.

Btrfs: fix 64 bit divide problem
This fixes a regression introduced by commit
cdcb725c05fe ("Btrfs: check
if there is enough space for balancing smarter"). We can't do 64-bit
divides on 32-bit architectures.
In cases where we need to divide/multiply by 2 we should just left/right
shift respectively, and in cases where theres N number of devices use
do_div. Also make the counters u64 to match up with rw_devices.

ALSA: hda - Don't spew too many ELD errors
Currently HD-audio driver shows the all error ELD byte as an error
in the kernel message. This is annoying when the video driver doesn't
set the correct ELD from the beginning. e.g. radeon sends a zero-byte
data, but we still check ELD with the fixed 128 byte as a workaround
for some broken devices, it spews 128-times errors.
For avoiding this, the driver aborts reading when the first byte is
invalid. In such a case, the whole data is certainly invalid.

ext4: flush any pending end_io requests before DIO reads w/dioread_nolock
There is a race between ext4 buffer write and direct_IO read with
dioread_nolock mount option enabled. The problem is that we clear
PageWriteback flag during end_io time but will do
uninitialized-to-initialized extent conversion later with dioread_nolock.
If an O_direct read request comes in during this period, ext4 will return
zero instead of the recently written data.
This patch checks whether there are any pending uninitialized-to-initialized
extent conversion requests before doing O_direct read to close the race.
Note that this is just a bandaid fix. The fundamental issue is that we
clear PageWriteback flag before we really complete an IO, which is
problem-prone. To fix the fundamental issue, we may need to implement an
extent tree cache that we can use to look up pending to-be-converted extents.

drm/i915: set GFX_MODE to pre-Ivybridge default value even on Ivybridge
Prior to Ivybridge, the GFX_MODE would default to 0x800, meaning that
MI_FLUSH would flush the TLBs in addition to the rest of the caches
indicated in the MI_FLUSH command. However starting with Ivybridge, the
register defaults to 0x2800 out of reset, meaning that to invalidate the
TLB we need to use PIPE_CONTROL. Since we're not doing that yet, go
back to the old default so things work.

In pcibios_get_phb_of_node(), we will crash while booting if
bus->bridge->parent is NULL.
